Website Privacy Policy and SMS Terms

Utah Navajo Health System, Inc. (“UNHS,” “we,” “us,” or “our”) respects the privacy of visitors to www.unhsinc.org. This policy explains how we collect, use, disclose, and protect information obtained through our website and through text-message communications.

Emergency and Crisis Notice

This website, its general contact forms, and ordinary email accounts are not continuously monitored and should not be used for emergencies.

If you or someone else is in immediate danger, call 911. If you are experiencing a mental health or suicide-related crisis, call or text 988 to reach the Suicide & Crisis Lifeline.

Relationship to Our HIPAA Notice

This Website Privacy Policy applies to information collected through the UNHS public website and related communications. It does not replace the UNHS Notice of Privacy Practices.

Information UNHS receives or maintains in connection with providing healthcare, behavioral-health, crisis-response, or Mobile Crisis Outreach Team services may constitute protected health information. UNHS handles protected health information in accordance with applicable law and the UNHS Notice of Privacy Practices.

Information We Collect

UNHS may collect information that you voluntarily provide, including:

  • Your name;
  • Telephone or mobile number;
  • Email or mailing address;
  • The UNHS service about which you are contacting us;
  • Information included in a contact form, text message, or other communication;
  • Communication preferences and records of SMS consent or withdrawal of consent;
  • Information needed to coordinate requested healthcare, crisis-response, or MCOT services; and
  • Other information you choose to provide.

When you visit our website, our service providers and we may also automatically collect limited technical information, such as your Internet Protocol address, browser type, device information, referring website, pages visited, actions taken, and date and time of access.

Please do not submit medical records, Social Security numbers, payment card information, or detailed medical information through the general website contact form or regular email. For patient-related communications, use the UNHS patient portal or contact the appropriate UNHS clinic directly.

How We Use Information

UNHS may use information to:

  • Respond to questions and requests;
  • Coordinate requested healthcare, behavioral-health, crisis-response, or MCOT services;
  • Send conversational and informational communications for which consent has been provided;
  • Provide appointment, scheduling, care-coordination, follow-up, wellness-check, or requested resource information;
  • Operate, secure, troubleshoot, and improve our website and communication systems;
  • Maintain records of communication consent and opt-out requests;
  • Protect patients, visitors, staff, and the public;
  • Comply with legal, regulatory, contractual, accreditation, and records-management requirements; and
  • Carry out other purposes authorized by the individual or disclosed when information is collected.

Text Messaging Through RingCentral

UNHS uses RingCentral as a communications service provider for certain voice and SMS/MMS communications.

UNHS text messages may include conversational or informational messages related to:

  • A request submitted to UNHS;
  • Mobile Crisis Outreach Team service coordination;
  • Appointment or scheduling information;
  • Care coordination and requested follow-up;
  • Wellness or safety check-ins;
  • Requested community resources; and
  • Responses to messages initiated by the individual.

The UNHS MCOT text-messaging program is not used to send marketing or promotional messages.

Providing a telephone number by itself does not constitute consent to receive SMS messages. Consent may be provided by selecting a separate optional SMS-consent checkbox, providing documented verbal or written permission, or initiating a text-message conversation with UNHS.

Consent to receive SMS messages is optional and is not a condition of receiving healthcare or other UNHS services.

SMS Disclosures

If you consent to receive conversational or informational SMS messages from Utah Navajo Health System, Inc.:

  • Message frequency may vary;
  • Message and data rates may apply;
  • Reply STOP to opt out of further messages;
  • Reply HELP for assistance; and
  • Consent may be withdrawn at any time.

After an individual opts out, UNHS will not send additional SMS messages from that number unless the individual subsequently provides new consent or sends an appropriate opt-in keyword such as START.

SMS delivery is subject to the availability and operation of wireless networks and telecommunications providers. UNHS and participating carriers cannot guarantee message delivery.

Text messaging is not an emergency-response system. Do not use SMS to report an emergency or rely on a text message when immediate assistance is needed.

Mobile Information and Consent

UNHS does not sell, rent, or use mobile numbers, SMS content, or SMS-consent information for marketing or promotional purposes.

No mobile opt-in or text message consent will be shared with third parties or affiliates.

RingCentral and applicable telecommunications carriers may process mobile numbers, message content, delivery information, and opt-out instructions on behalf of UNHS only as necessary to transmit and support the requested communications, maintain security, enforce messaging preferences, and comply with applicable law.

When Information May Be Disclosed

UNHS may disclose information:

  • To authorized UNHS employees and workforce members who need it to perform their duties;
  • To contracted service providers supporting website hosting, communications, security, analytics, or healthcare operations;
  • To healthcare providers, crisis personnel, or emergency responders when permitted by law and reasonably necessary to provide requested services or address an immediate health or safety concern;
  • When required or permitted by law, court order, regulatory requirement, or governmental request;
  • To investigate fraud, misuse, security incidents, or threats to individuals or systems; or
  • With the individual’s authorization.

When a service provider handles protected health information on behalf of UNHS, UNHS requires appropriate privacy and security protections, including a Business Associate Agreement when required.

UNHS does not sell personal information.

Cookies and Website Analytics

UNHS may use cookies, Google Analytics, Google Tag Manager, server logs, or similar technologies to support website functionality, security, and general usage analysis.

UNHS does not intend for website analytics tools to collect medical records, crisis communications, detailed health information, or information entered into secure patient-service systems.

Visitors may limit cookies through browser settings, although blocking cookies may affect certain website functions.

External Websites and Services

Our website may contain links to external websites and services, including the patient portal, RingCentral, crisis resources, forms, and community resources.

The privacy terms of an independently operated website apply after a visitor follows an external link. UNHS is not responsible for the privacy or security practices of independently operated third-party websites.

Information Security

UNHS uses reasonable administrative, physical, and technical safeguards designed to protect information under its control. However, no website, email system, text-messaging system, electronic transmission, or storage system can be guaranteed completely secure.

Individuals should consider who may have access to their telephone, whether messages appear on a locked screen, and whether others share access to the device before choosing to communicate about sensitive matters by SMS.

Information Retention

UNHS retains website and communication information only as long as reasonably necessary for the purposes described in this policy and to satisfy healthcare, legal, security, records-management, and operational requirements.

Medical and behavioral-health records may be subject to separate retention requirements and may not be eligible for deletion when UNHS is legally required to maintain them.

Your Choices

You may:

  • Decline to consent to SMS communications;
  • Withdraw SMS consent by replying STOP;
  • Request assistance by replying HELP;
  • Unsubscribe from newsletters using the instructions in the message;
  • Control certain cookies through browser settings; and
  • Exercise rights concerning protected health information as described in the UNHS Notice of Privacy Practices.

Changes to This Policy

UNHS may update this policy as its services, technology, or legal obligations change. The revised policy will be posted on this page with an updated effective date.
